Procedure Violations.

i-On October 1,1973, the motor-operated valves 2BS-1 and 2BS-2 in the reactor

~ building spray system were closed and the notor operator breakers were opened and white tagged to prevent inadvertent initiation of the spray system.

They were' tagged a second time on October 5, 1973 for Engineering Safeguards testing. During the period October 17-24, 1973, maintenance was' performed on the 2B building spray pump to repair a seal leak. On October 26, 1973, the performance of the reactor building spray pump 2B was checked using the reactor building spray system performance test PT/0/A/204/7.

Step 12.11 of

- the procedure requires that valve 2BS-2 be closed and its breaker be white tagged open.

Since this valve aircady had two white tags issued to it in this position, the operator signed the procedure step as having been completed, without issuing his own tag for the required test. After completion of the test,.the operator left the existing tags and signed off the procedure. step

requiring removal of his white tag.

To avoid recurrence of a similar situation, the Oconee operating engineer reviewed this incident with the shift supervisors and they in turn with all shift personnel.

Strict adherence to procedures regardless of whether they 1

think that other actions have covered a step was emphasized to these personnel.

1 Furthermore, an additional step has been added to the reactor building spray i

j system performance test procedure, requiring the' test coordinator to personally verify that the breakers on-2BS-1 and 2BS-2 have been racked in or that he 3

-has personally notified the shift supervisor that they are still open for reasons other than for the test.

I. A.1.d Reactor Coolant System Heatup Limitations The Station Review Committee reviewed this incident again during its meeting of December 18, 1973. The committee concluded that either -reactor coolant 1

system heatup or depressurization would have been appropriate; however, 1%

reduction of reactor coolant pressure would have returned the system within the heatsp limits r;ooner and would have been the best course of action to take.,

The Station Review Committee minutes of December 18, 1973 should provide sufficient information to close this item.
