Occurrence Date: May 12, 1976 Facility: Oconee Unit 2, Seneca, South Carolins Identification of Occurrence: Operation of polar crane without approved procedure Conditions Prior to Occurrence: Unit in refueling shutdown Description of Occurrence:

On May 12, 1976, while Oconee Unit 2 was in refueling shutdown, the polar i

crane was used to hoist the Main Fuel Handling Bridge Inner Control Rod Mast for maintenance purposes. An approved procedure was not used, contrary to the requirements of Oconee Technical Specificacian 3.12.3.

Specification 3.12.3 requires that when the reactor vessel head is removed and irradiated l

fuel is in the Reactor Building, the polar crane shall be operated only in accordance with approved operating procedures.

Apparent Cause of Occurrence:

1 Personnel involved in this incident were not fully aware of requirements to follow an approved procedure when operating the polar crane under the I

conditions as stated in Oconee Technical Specification 3.12.

Analysis of Occurrence:

Although an approved procedure was not utilized, the polar crane was not operated in an unsafe manner.

The lift made involved raising the inner control rod mast approximately ten feet above the Main Fuel Handling Bridge which_was " parked" 25 feet from the center line of the reactor vessel.

Had the load broken loose, the Control Rod Mast would not have fallen near the j

vessel or any irradiated fuel.

It is concluded, therefore, that this incident did not adversely affect the health and. safety of the public.

Correctivs_fetion:

All supervisors, engineers, and crane operators, including the responsible personnel involved in this incident, have been made. aware of the requirements j

for operating cranes only in accordance with proper procedures as specified in Oconee Technical Specifications'3.12.. Additionally, by June 1, 1976,

. signs will be placed on each ladder to the polar cranes and in the auxiliary hoists cautioning personnel of procedural requirements necessary for operation.
