Response to IE Inspection Report-50-269/80-28, -270/80-24, -287/80-21 i

As required by 10CFR 50, Appendix B, Criteria VIII and as implemented in Section 17.2.8 of Duke Topical Report lA, Quality Assurance Program, measures to maintain material traceability and accountability shall be utilized.
Contrary to the above, accountability and traceability of material was not maintained on material used in the performance of work documented by j'
station work request 48231 (specifically bolting material used in mechan-ical joints on an instrument manifold root valve on RCP 1A2).

-RESPONSE l
This item is correct and is the result of a personnel error.
An individual in the materials issue area failed to properly control and issue the subject l
material.
The immediate corrective actions taken consisted of the following:
1.
The stud material in question was verified to be of the correct material.
2.
Material control instructions were reviewed with all appropriate personnel.
3.
Appropriate disciplinary action was taken against the individual who failed to properly control th-QA material. Retraining was conducted with the individual and verified by testing.
Additional correction actions to'be taken to prevent reoccurrence are:
1.
The materials traceability program will be revised to provide better control and documentation. This revision will be completed by January 1, 1981.

Written instructions will be revised to better define to personnel the 4
methods to control and document material traceability by December 1, 1980.

The appropriate station directive will be reviewed by materials personnel emphasizing disciplinary action will result from failure to follow the station directives. This action will be' completed by November 1, 1980.

ITEM Technical Specification 6.4.1 requires the station be operated and maintained in accordance with current, written, approved procedures containing appropriate checkoff lists and instructions.
Contrary to the above, on July 21, 1980, license personnel performed radioactive waste maintenancc functions without utilining procedure HP/0/B/1600/12 which led to a spill of radioactive material.

RESPONSE
l This item is correct and is thi result of personnel errors.
The Maintenance Planner involved specified precedure MP/0/A/1600/05, Removal and Replacement j
of the Letdown Storage Filter, on the job work request instead of procedure MP/0/A/1600/12, Letdown Storage Filter Transfer Cask Unloading. This mistake was not recognized by either the Maintenance Supervisor or the Maintenance Technician involved; therefore, the wrong procedure was used for the job.
The procedure used, MP/0/A/1600/05, does not specifically address movement and f'
loading of the cask.
Immediate corrective action was to contain and clean up the small spill of radioactive material.
The Maintenance Technician and rianner involved were counseled about their mistakes in this incident.
A structural change to the inclined ramp at the cask loading area is being made by the Mechanical Maintenance Section. This change should help prevent the electric transport cart from overturning as it did in this instance. This work should be complete by December 1, 1980.
In addition, advinistrative changes are being made to improve follow-up on work done by the Maintenance Planners. Additional training will be conducted for all supervisory personnel I
in the Maintenance Group ss part of the on-going training program. A schedule for this training will be finalized by January 1, 1981, i
