.o APPENDIX A NOTICE OF VIOLATION Florida Power Corporation License No. DPR-72 C ystal River Nuclear Plant Based on the NRC inspection June 2 - July 31, 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indi-cated below. These items have been cati:gorized as described in correspondence to you dated December 31, 1974.

A.

Environmental Technical Specification 3.1.5 states in part:

"Representa-tive samples shall be obtained and analyzed once per month from test wells 1, 4 and 5, the discharge canal directly to the North of the ponds, at the ponds themselves. (See Figure 3.1-1)."

Figure 3.1-1 illustrates the locations of test wells 1, 4 and 5.

10CFR Part 50.59(a)(1) states: "The holder of a license authorizing opera-tion of a production or utilization facility may (1) make changes in the facility as described in the safety analysis report, (ii) make changes in the procedures as described in the safety analysis report, and (iii) conduct tests or experiments not described in the safety analysis report without prior Commission approval, unless the proposed change, test or experiment involves a change in the technical specifications interporated in the license or an unreviewed safety question."

Contrary to the above, as of February 22, 1980, samples have not been obtained and analyzed from test well 4, and on March 13, 1980, a change to Environmental Technical Specification 3.1.5 was made to eliminate test well 4 without prior Commission approval.

This is an infraction.

B.

10CFR Part 50, Appendix B, Criterion V states in part: " Activities affecting quality shall be prescribed by documented instructions, procedures, or.

drawings, of a type appropriate to the circumstances..."

FSAR Section 17.1 states in part:

" Florida Power Corporation has a total QUALITY PROGRAM to establish and maintain standards of quality covering the iesign, procurement, manufacturing, construction, testing, operations, naintenance, refueling, and modifications of applicable structures, com-ponents, and systems for Crystal River _ Plant Unit 3... This program is 1

directed toward meeting the intent of the requirements of NRC Regulation' 10CFR Part 50, Appendix B, " Quality Assurance Criteria for Nuclear Power Plants."

Contrary to the above, on July 18, 1980, an operational event occurred which resulted in the loss of pressurizer level, due to the use of an inadequate procedure.

This is an infraction.
