This refers to the telephone conversation between you and Mr. E. J. Brunner of this office on October 1,1980, regarding the Reactor Trip and Resulting Transient at Pilgrim Nuclear Power Station, Unit 1, earlier that date.
With regard to the matters discussed, we understand that you will:
1.
Determine and correct the specific problems which caused the "D" Safety Relief Valve to stick, open and the Recirculation Pumps to fail to runback.
2.
Perform a thorough investigetion to identify the causes of the Main Steam Une High Radiation Level trip and the inability to restart the "A" and "C" Reactor Feedwater Pumps, following that trip; including a review of personnel performance and procedures adequacy as they relate to these events.
3.
Maintain the plant in a Cold Shutdown condition until the corrective actions specified in item 1 above are complete.
