APPENDIX A NOTICE OF VIOLATION Alabama Power Company License Nos. CPPR-86 Farley Nuclear Plant
& NPF-2 Based on the NRC inspection June 17-20, 1980, certain of your activities were r
apparently not conducted in full compliance with NRC requ1rements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
A.
As required by 10 CFR 50,. Appendix B, Criterion V, as implemented by FSAR Section 17.1.5, activities affecting quality shall be prescribed by docu.
mented instructions, procedures or drawings...and shall be accomplished in accordance with these instructions, procedures, or drawings.

FNP QC procedure 4.01 paragraph 3.0 requires that inspection parameters required by the designer for stress analysis will be identified in the inspection packages and shall be measured and recorded.
Contrary to the above, Boron transfer piping shown on drawing SK-G12-CV-210 was in contact with piping shown on SK-P12-CV-209. This condi-tion was neither inspected for, nor reported.

FQC procedure 5.4.2.1 Addendum 1 paragraph. 5.1.2.2 requires QC to 2.
document hanger discrepancies on a hanger discrepancy and correction report.
Contrary to the above, discrepancies noted for safety injection piping hanger 2SI-12130 were documented by QC on a piece of tablet paper rather than a hanger discrepancy and correction report.
This is a deficiency. This applies to CPPR-86 only.
B.
As required by 10 CFR 50, Appendix B, Criterion V, as implemented by FSAR Section.17.1.5, activities affecting ' quality shall be prescribed by docu-mented instructions,' procedures or drawings...and shall be accomplished in accordance with these instructions, procedures or drawings. Construction Work Request No.1-80-9110 required modification and inspection of support CVC-R254 in accordance with QC procedure 5.4.2.1 Addendum 1.
Contrary to the above, 'QC ' procedure 5.4.2.1 Addendum 1 did not provide adequate instruction for the modification and inspection of the support.
Procedure. FNP-1-GMP-43 Appendix "D" requirements were used for the instal-lation and inspection but records for the support do not reflect this.
This is a deficiency. This applies to NPF-2 only.

As required by 10 CFR 50, Appendix B, Criterion XII, and as implemented by FSAR Section 17.1.2, measures shall be established to assure that tools, gages, instruments.arJ other measuring and testing devices used in activities affecting quality are properly controlled, calibrated and adjusted at
-specified periods to maintain accuracy within necessary limits.
Contrary to the above, the torque wrench multiplier used for IEB 79-02 testing of support 2SW-R119 was not controlled nor calibrated. A multiplier that was checked on 6-20-80 for the inspector showed it provided a resultant torque that-was.approximately-12 percent less than the required resultant torque. ;The licensee indicated-that-control and calibration of torque wrench multipliers was not required.
This is an infraction.
