Narrative

Report for LER 50-366/1980-105.

During normal operation on August 21, 1980, nitrogen pressure was lost to both Stand-by Liquid Control System Accumulators, 2C41-A003A & B.

The SBLC systems were administratively declared inoperable as per Tech Specs 3/4.1.5.

The cause of the event was the accumulator " A" nitrogen pressure b'eing found at less then 450 psi and nitrogen pressure being lost to accumulator "B" while performi.ng Stand-by Liquid Control System Pump Operability Test and Relief Valve Test, HMP-2-3702.

Investigation of the problems revealed that the accumulator "A" test valve, 2C41-F003A, was leaking and accumulator "B" test valve, 2C41-F003D, had been broken while installing a test gauge during the surveillance. Within one hour of discovery new test valves were installed, accumulators recharged and the system returned to service.

i The purpose of the SBLC accumulators is to dampen the pressure surges of the l

positive displacement SBLC pump.

It is our contention that although the SBLC l;

system has not been analyzed to function without accumulator pressure, the ll system would have performed its design function.

The generic review revealed i

no inherent problems. This is a non-repetitive event and there were no effects on public health or safety due to this event.
