Gentlemen:

This refers to the inspection conducted by Messrs. J. D. Smith and W. G.

Guldemond of this office during the period June 4 through 27, 1980, of

activities at Point Beach Nuclear Plant, Units 1 and 2 authorized by NRC

Operating License Nos. DPR-24 and DPR-27 and to the discussion of our

findings with Mr. J. J. Zach at the conclusion of the inspection. This

also refers to the Enforcement Conference held with you and members of your

staff on July 8, 1980.

The enclosed copy of our inspection report identifies areas examined

during the inspection. Within these areas, the inspection consisted of a

selective examination of procedures, observations, and interviews with

personnel.

During this inspection effort, certain of your activities were found to

be in noncompliance with NRC requirements, as described in the enclosed

Appendices A and B.

You should note that Item 1 of Appendix A, which

involves the loss of both RHR trains, literally meets the threshold of

the violation category of noncompliance. However, in evaluating the

seriousness of this matter (particularly the facts that both systems were

out of service a maximum of 90 seconds and it was not likely the noncom-

pliance could have continued for a longer period), we have categorized

this item as an infraction.

This notice is sent to you pursuant to the provisions of Section 2.201 of

the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations.

Section 2.201 requires you to submit to this office within twenty days of

your receipt of this notice a written statement or explanation in reply,

including for eacia item of noncompliance: (1) corrective action taken and

the results achieved; (2) corrective action to be taken to avoid further

noncompliance; and (3) the date when full ccmpliance will be achieved.

As discussed with you and members of your staff on July 8, 1980, we are

concerned over the increased number of persannel errors experienced more

recently at Point Beach.

Certain of thc.e errors resulted in reduced

margins of safety in that safety related equipment was inadvertantly

removed from service. These errors are untypical of the high level of

discipline that has characterized the Point Beach operation in the past

and appear to be related to the recent loss of key plant personnel.

Furthermore, we note that certain of these errors involved failure to

communicate important informatioa to all involved individuals.

In your

response to this letter please describe the actions you are taking to

improve control of safety related equipment by licensed personnel and to

improve communications between plant personnel to assure awareness of

equipment out of service and awareness regarding scheduled surveillance

and maintenance activities.

Certain areas examined during this inspection concern a subject matter which is

exempt from disclosure according to Section 2.790 of the NRC's " Rules of

Practice," Part 2, Title 10, Code of Federal Regulations. Consequently,

Appendix B to this letter, an enclosure to our report of this inspection,

and your response to the noncompliance identified in the subject appendix

  • o this letter will not be placed in the Public Document Room. Therefore,

your statement of corrective action regarding the noncompliance identi-

fied in th3 enclosure should be submitted as a separate enclosure to your

transmittal letter.

In addition, if the enclosures contain information

that you or your contractors believe to be proprietary, you must apply in

writing to this office, within twenty days of your receipt of this letter,

to withhold such ir

,mation from public disclosure. The application

must include a ful statement of the reasons for which the information is

considered proprietary, and should be prepared so that proprietary informa-

tion ident'.fied in the application is contained in an enclosure to the

application.
