r feed pump. turbine control, forcing the operator to trip a-recirculation: pump ~

t in order to reduce. power and recover vessel level.. The feed pumpLturbine control system.was repaired'and the plant returned to normal; full _ power Joperation on-November 18,;1989.

On' November-25, a failure of'the post-filter assembly of the "B" instrument:

- air dryer caused-a'l'oss.of plant air supply.-.As main steam isolation valves (MSIVs)' started _. drifting closed, as_a result of the decreasing air' pressure, lthe MSIV not full-open signal triggered an automatic scram. The plant-remained shutdown'through the_end of' November wh11e' instrument air dryer-

-repairs were completed and system testing was-performed.

  • An automatic' reactor scram occurred due to MSIV drift and subsequent closure trip. signal when an instrument air dryer post-filter element caught' fire and the post-filter vessel f ailed, resulting in loss of instrument air.

The dryer- temperature . limit switch.was determined to be faulty during instru--

ment-air l system troubleshooting and' testing. _Both the switch and post-dryer filter were' replaced.during sub-sequent maintenance. The post-dryer.

filter media was uncraded to'n hich temperature element.
