SUBJECT:

NOTICE OF VIOLATION

(NRC INSPECTION REPORT NOS. 50-327/89-25 AND 50-328/89-25)

.

This refers to the Nuclear Regulatory Commission (NRC) routine monthly

inspection conducted by D. P. Loveless from October 6,198S thru November 8,

1989.

At= the conclusion of the inspection, the findings were discussed with

those members of your staff identified in the enclosed inspection report.

Areas examined during the inspection are identified in the report.

These areas

. included operational safety verification and maintenance and surveillance

activities.

Within these areas, the inspection consisted of selective

examinations of procedures and representative records, interviews with

personnel, and observation of activities in progress.

The inspection findings indicate that certain activities appear to violate NRC

requirements.

The identified violations involved two instances of failure to

,

follow surveillance procedures and one instance of failure to promptly classify

!

an event 'as a Notification of Unusual Event.

The violations, references to

pertinent requirements, and elements to be included in your response are

l

described in the enclosed Notice of Violation.

!

The third violation described in the enclosed Notice is similar to a violation

contained in the Notice sent to you by our letter dated August 22, 1988.

Because "similar violations," as described in the NRC Enforcement Policy, are

of significant concern to the NRC, please give particular attention in your

response to the identification of the root cause of this problem and your

i

corrective action to preve.it recurrence.
