4 ENCLOSURE PILGRIM STAlVS REPORT FOR THE PERIOD NOVEMBER 4-17, 1989 1.0 Plant Status' As of 8:00 a.m. on Novemb(r 17, 1989, the plant was. operating at approxi-mately 94% power with the turbine generator. synchronized to the grid.
2.0 power Ascension Test Pr9 gram Status At the end of the. list repsrt period, the plant was in cold shutdown while the licensee comme'.ced pressurization of the reactor coolant pressure boundary in order to perform primary system leakage testing, cs well-as instrument line flow check valve testing.
Primary system leakage testing was performed to verify the integrity of the reactor coolant pressure boundary. The reactor instrumentation excess flow check valves were tested for seal ieakage to ensure minimum reactor ecolant pressure boundary leak-age in the event of an instrumentation line failure.
These tests were completed successfully ca November 4, 1989.
At 12:08 p.m. on November 6, 1989 the licensee brought the reactor critical following completion of the scheduled October mini-outage. The turbine
. generator was synchronized to the electrical pcwer distribution grid at 5:43 p.m. on November 8, 1989. At the close of this~ report period, the plant was at 94% power and was limited to that power due to a lockup of the "A" recirculation motor generator set scoop tube. This inhibited the oper-ators!. ability to adjust recirculation flow.
In a boiling water reactor such as Pilgrim, the reactor recirculation system provides forced.circula-tion of reactor coolant through the core to enable the reactor to achieve L
a higher power output than would be possible without forced circulation.
The licensee is continuing to troubleshoot this problem. Detailed NRC re-i-

The licensee continued to conduct routine surveillances-along with preven-l
.ive and corrective maintenance throughout the period.

-3.0 Emergency Notification System (ENS) Reports Group I Containment 1 solation Uue to High Reactor Vessel Water Level On November 6, 1989 the licensee experienced an isolation of the main steam isolation valves, main steam line drain valves and the recirculation system sample valves (a Group I primary containment isolation) due to high reactor vessel water level. The high water level occurred while the licensee was restoring the feedwater system to a normal lineup.
The licensee determined that the cause of the event was a mispositioned manual startup feedwater regulating valve. At the time of the event, the plart was in the startup mode. Effect on~the plant was minimal and the licensee successfully reset the isolation. The licensee reported this event to the NRC via the ENS.

f Inadvertent Secondary' Containment Isolation On' November 11, 1989 the licensee experienced an inadvertent partial isola-tion of'the reactor building isolation system.
This unnecessary actuation occurred during surveillaar testing of the reactor building isolation logic system..The license, determined that a loose wire clip caused the actuation..The wire clip was reattached to the relay and the isolation was reset. The reactor building solation system is designed to initiate auto-matic isolation of the reactor building to provide timely protection against' the' release of radioactive materials.

- I, 4.0 Pilgrim Restart Staff Activities l
The' Pilgrim Restart Staff was in extended shif t coverage at the beginning
]
of this report period. At 10:00 p.m. on November 5, 1989 the Restart Staff l
began around-the-clock shift coverage in anticipation of plant startup.
Around-the-clock shift coverage continued until 2:00 p.m. on Novembrr 10, 1
-1989. At.the close of this report period, the Restart Staff was in ex-l tended shift coverage, consistent with reduced testing activity.

The Pilgrim Restart Staff continued to evaluate the performance of licensee personne1~and the. plant. An NRC-developed inspection plan addressing the 100% power plateau, as well as the primary functional areas of operations, maintenance, surveillance, and startup testing was implemented. The..in-
.spectors assessed operator performance, organizational interfaces, ad-herence-to procedures,.and Technical Specification compliance.
