monthly surveillance and maintenance observations, onsite follow 4 of events,

access control, and emergency exercise evaluetion. Within these ar ns, the

inspection consisted of selective examination of procedures and representative
records, interviews with personnel, and observations by the inspectors.
The
'
inspection findings are documented in the enclosed inspection report.
During this inspection, it was found that certain of your activities were in
violation of NRC requirements.
Consequently, you are required to respond to
these violations, in writing, in accordance with the provisions of
Section 2.201 of the NRC's " Rules of Practice," Part 2, Title 10. Code of
Federal Regulations.
Your response should be based on the specifics contained
in the Notice of Violation enclosed with this letter.
The enclosed violation is a repeat of one discussed in NRC Inspection
Reports 50-298/88-31 dated November 18, 1988, and 50-298/89-07 dated March 20,
1989.
At that time the NRC expressed a concern about management attention to,
and involvement in, the area of training.
The repetition of a violation raises
,
questions about the adequacy of your corrective actions program and your
responsiveness to NRC concerns.
This area was identified as needing
improvement in the latest SALP.
The NRC remains concerned that NPPD is not
providing adequate management attention to the training area.
This is further
evidenced by growing concerns about your requalification program as discussed
in the NRC examination report issued by this office on October 16, 1989.
We have also examined actions you have taken with regard to previously
identified inspection findings.
The status of these items is identified in
paragraph 5 of the enclosed report.
