This refers to the inspection conducted by Messrs. W. F. Smith,' T. R. Staker,
and S. D. Butler of this office during the period October 1-31, 1989, of

activities authorized by NRC Operating License NPF-38 for Waterford Steam
' Electric Station, Unit 3, and to the discussion of our findings with
Mr. J. R. McGaha and other members of your staff at the conclusion of the

inspection.

Areas examined during the inssection included plant status, onsite followup of
events, monthly maintenance oaservation, monthly surveillance observation.
[
operational safety verification, licensee event report followup, installatton
t
and testing of modifications, and refueling activities. Within these areas,
r
'
the inspection consisted of sr.lective examination of procedures and
<
representative records, interviews with personnel, and observations by the NRC
inspectors. The inspection findings are documented in the enclosed inspection
report.
During this inspection, it was found that certain of your activities were in
violation of NRC requirements. Consequently, you are required to respond to
.
this violation, in writing, in accordance with the provisions of Section 2.201
I
of the NRC's " Ruler. of Practice," Part 2. Title 10. Code of Federal
,
Regulations. Your response should be based on the specifics contained in the
<;

Notice of Violation enclosed with this letter.
In accordance with NRC
.
Enforcement Policy, .this Level V violation would not normally have been
cited.
It was cited in this instance because it is believed that there may be
a weakness'-in your program for accountability of valves repositioned for
maintenance that are not inrediately tagged. Your response to this violation
i
should also address this p0tential weakness.

During this inspection, a review of your response as documented in licensee
.
event = reports (LERs) 50-382/89-014 and 50-382/89-017 revealed the need for
.
supplemental information. These LERs are documented in paragraph 8.b of this
i
report. This was discussed with Mr. S. R. McGaha on November 27, 1989, and it
i
- was agreed that supplemental LERs would be provided.
One unresolved item is identified in paragraph 3.b of the enclosed inspection
report, and one is identified in paragraph 3.c.
