The inspection determined that weaknesses in administering and controlling

-maintenanceatTrojanwasamajorrootcauseoftheapparentviolations.

These weaknesses were in the areas of work reviews, work instructions, design

-basis. knowledge, and shift turnovers. The inspection also identified that the

-Shift Supervisor and the Assistant Shift' Supervisor spend a large portion of

their on-shift time initiating, reviewing and authorizing maintenance

requests.. We understand that you are evaluating changes to that practice. We
