Dear Mr. Michaels:

'An error was made on one of the data pages contained in Appendix B to the l Subject Report. The results of the " Survey for Loose Surface Contamination  !

' at Max. Point (f)" in Room 19? (Health-Physics ; Lab.) are incorrect (see the-second-to-last page of the Report). When the data from the actual survey L forms was entered into the computer date-base that was used to generate the summary data and plots, the values for alpha and beta-gamma counts were transposed. This resulted in reporting incorrect (high) results for maximum-alpha and slightly lower t.~a actual average / maximum beta-gamma loose-surface contamination.-

The error has since been corructed and 'a new summary page (attached) was generated.

Please intert the attached " corrected" page and remove / discard the old (incorrect) one. Thank you for your cooperation and understanding.
