- Gentlemen:
This refers to-the investigation conducted by Messrs. J. B. McCarten and
N. J. Chrissotimos of this office on December 5,1979 through January 15,
1980, of activities at the Quad-Cities Nuclear Power Station, Unit 1,
authorized by NRC License No. DPR-29 and to the discussion of our find-
ings with Mr. N. J. Kalivianakis and others of his staff at the conclu-
sion of this investigation.
This investigation was conducted to determine the circumstances surround-
ing the licensee's discovery that two valves in the core spray system of
Unit 1 were found to'be in a closed position.
Within these areas, the
investigation consisted 'of a ' selective examination of procedures and re-
presentative records, observations, and interviews with personnel.
No items of noncompliance with NRC requirements were identified during
'the course of this investigation, nor was any conclusion reached as to
the manner in which the valves in the core spray system were placed in
the closed position. We are concerned, however, that glass coverings
on the control boxes over switches in the 1A and 1B core spray rooms

apparently had-be'en broken for some time allowing unimpeded access to
the switches.
This obviously defeated the purpose of having the
switches within a locked control box. Please respond informing us of
actions you will-take or'have taken to assure that the integrity of
such locked ' switch boxss is maintained.
In accordance with Section 2.790 of the NRC's " Rules of Practice," Part

2, Title 10, Code of Federal Regulations, a copy of this letter and the

enclosed investigation report will be.placed in the.NRC's Public Document
Room, except.as follows.
