Y APPENDIX A NOTICE OF VIOLATION Tennessee Valley Authority License No. CPPR-73 Sequoyah Based on the NRC inspection May 19-22, 1980, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.
As required by 10 CFR 50, Appendix B, Criterion V, and implemented by Paragraph 17.1A.5, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instruc-tions, procedures, or drawings." TVA's ultrasonic examination procedures N-UT-1, Rev. 3, and N-UT-6, Rev. 1, require scan limitations and wall thickness measurements te be recorded.
Contrary to the above, ultrasonic examination personnel have not recorded scan limitations, nor does the procedure provide a space on the report form for recording such data.
This is a Deficiency.
