80 INSPECTOR REPORT - RESPONSE TO DEFICIENCY The subject letter dated June 25, 1980, cited TVA with one deviation in accordance with 10 CFR 2.201.

ENCLOSURE SEQUOYAH NUCLEAR PLANT UNIT 2 RESPONSE TO NRC DEFICIENCY FAILURE OF ULTRASONIC PROCEDURE TO ADEQUATELY PROVIDE INSTRUCTIONS AND FAILURE OF THE ULTRASONIC EXAMINERS TO COMPLY WITH THE PROCEDURE Deficiency 50-328/80-11-02 As required by 10 CFR 50, Appendix B, Criterion V, and implemented by Paragraph 17.1A.5, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these initructions, procedures, or drawings." TVA's ultrasonic examination p ocedures N-UT-1, Rev. 3, and N-UT-6, Rev. 1, require scan limitations and wall thickness measurements to be recorded.

Contrary to the above, ultrasonic examination personnel have not recorded scan limitations, nor does the procedure provide a space on the report form for recording such data.

Corrective Action Taken and Results Achieved The NDE examiners were cautioned and reminded that any and all failure to obtain complete scans must be documented on the report of ultrasonic examination.

Steps Taken to Avoid Further Recurrence The report of ultrasonic examination will be revised to include a space to document incomplete scans.

Date of Full Compliance The NDE examiners were cautioned immediately af ter the NRC inspector brought the concern to TVA's attention. The report of ultrasonic examina-tion data sheet will be revised by August 29, 1980. TVA will be in full compliance by September 5, 1980.
