Dear Mr. O'Reilly:

On August 1,1980 a report was made under the provisions of 10CFR50.55(e) concerning the possibility that the Bailey Control Company current or voltage buffer cards, PfiS624610, could be installed improperly in the 820 buffer modules in the power channels of the Essential Control Instrumentation (ECI) Systems.

In accordance with the reporting requirements of 10CFR21, the folicwing information is submitted:

It has been determined by the. Babcock & Wilcox Company that it is possible to interpret a statement in the Bailey Control Company product Instruction Book E92-79 for inputting buffer cards for the ECI Systems to mean that these cards may be replaced without remov-

ing the buffer module. In doing so, it is possible, during  ;

field changes, to improperly install at least one card in each '

of the power channels resulting in a potential loss of power to both channels. By removing the buffer module prior to ,

installation of the card, a visual inspection can be conducted to insure that the card has been installed properly.

Babcock 8 Wilcox reported this problem to Victor Stello, Director, Office of Inspection and Enforcement, United States Nuclear Regulatory Commissicn, via a letter dated July 18, 1980. In this letter, Babcock '

& Wilcox recommended rewording the maintenance instructions in the Bailey Centrol Company product Instruction Book E92-79 to require removal of- the buffer modules prior to installation of the beffer cards so as to insure proper installation of the cards. The reworded in-struction book would be provided to VEPCO as soon as it is available.

VEPC0 is currently evaluating the Babcock & Wilcox recommendations

and we will consider this to be the final report on this subject unless 4 it is decided to implement corrective actions above and beyond those recommended by Babcock & Wilcox.
