Documentation in support of that appointment, such as a curriculum vitae or a letter of appointment, identifying information, such as her education, experience, and citizenship as required by Title 10 of the Code of Federal Regulations, Part 70.22(a)(1).
A chart which identifies the organizational relationship of the senior university official for the use of radioactive material to university management and to the office of the Radiation Safety Officer.
