Appendix A NOTICE OF VIOLATION Commonwealth Edison Company Docket No.

50-254 Based on the results of the NRC inspection conducted on July 9-11, 1980, it appears that certain of your activities were not conducted in full compliance with NRC requirements, as indicated below.

Item A is con-sidered an infraction and Item B a deficiency.

A.

10 CFR Part 70.51(c) requires the licensee to establish, maintain and follow written material control and accounting procedures which are sufficient to enable the licensee to account for the special nuclear material in his possission under Licensee.

(1) Contrary to the above, the licensee's procedures were not sufficient to physically account for and control fission counters as a part of physical inventory. As a result of an inventory verification conducted by the inspection team, one of eight fission counters (dunkers) was discovered to be missing. This General Electric fission counter serial number 65.84075, contained approximately 1.9 grams of uranium 235 enriched at 93%.

10 CFR 70.51(b)(1) states that each licensee shall keep records showing the receipt, inventory, (including locations), disposal, acquisition, and transfer of all special nuclear material in his possession, regardless of its origin or cethod of acquisition.

(1) Contrary to the above, the update of the Fuel Location and Exposure records (Form E) was not correctly accomplished and reflected many fuel assemblies in the Unit 1 fuel pool locations, when in fact, on June 3, 1990, these fuel assem-blies had been transferred from the Unit i fuel pool to the Unit 2 fuel pool locations.
