Dear Mr. Keppler:
Toledo Edison acknowledges receipt of your June 25, 1974 letter and enclosure referencing an apparent violation of AEC requirements and nonconformances with our quality assurance program. The item resulted from a site inspection
[s conducted by your office on June 4 - 5, 1974.
G Following a thorough examination of the, item of concern, Toledo Edison offers the following information regarding the apparent violation, including corrective actions and steps taken to avoid further repetition.
Item A - Documented Procedures - Welding Instruction Sheet - Criterion V The use of a Babcock & Wilcox Construction Company (B&W) unapproved Weld In-struction Sheet noted during the June 5, 1974 AEC inspection has been documented by B&W as nonconforming.
B&W Field Construction Procedure Deviations recommend-ing " accept-as-is" disposition have been submitted and have received appropriate reg.uited approvals. An audit of construction records has been performed by B&W quality control personnel followed by a B&W quality assurance audit confirming this occurrence as an isolated incident.
To preclude recurrences of this nature, B&W has reemphasized with their personnel the provisions of the quality control program regarding use of only current, approved procedures.
The Bechtel quality control, mechanical, piping, welding, and electrical engineers have stepped up their inspection and surveillance activities and are working closely with the applicable contractor's approved quality programs and procedures.
Bechtel has increased their inspection staff and are conducting QC/QA Training Programs emphasizing the Davis-Besse QA Program requirements.

Following this inspection visit, Toledo Edison Management held separate QA meetings with both B&W and Bechtel top project management concerning the quality assurance activities of the project and stressing the need for more attention to be given to the details of quality assurance activities.
Both B&W and Bechtel have responded in a very positive manner and there is definite improvement on their part in all areas concerned. Toledo Edison has increased its efforts in review of the site quality assurance activities and has maintained close monitor-ing and surveillance of the actions described herein.
