DATE'OF EVENT: June 18, 1978 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Containment Spray Pump 1-2 inoperable.

Conditions Prior to Occurrence: The unit was in Mode 6, with Power (MWT) = 0, and Load (MWE) = 0.

Description of Occurrence: On June 18,1978 at' 1015 hours0.0117 days <br />0.282 hours <br />0.00168 weeks <br />3.862075e-4 months <br /> during performance of

  • Surveillance Test ST 5031.07, " Safety Features Actuation System 18 Month Refuel-ing Test", Cont =4=ent Spray (CS) P :mp 1-2 failed to start automatically on sequence. The pump was thus inoperable.

Technical Specification 3.3.2.1 requires the operability of both CS Pumps while in Modes 1, 2, 3.and 4.

Since the unit was in Mode 6 at the time of the occur-rence, Action Statement 9 was not applicable. This occurrence is being reported as documentation of component failure.

- ' Designation of Apparent Cause of Occurrence: The cause of the occurrence was a component failure. The failure of Safety Features Actuation System (SFAS) output module L-41,2 caused failure of relays to de-energize.

Analysis of Occurrence: There was no danger to the health and safety of the public or unit personnel. One other Containment Spray Pump,1-1, was op'erable during the periods that CS 1-2 was inoperable. The unit was in Mode 6 at the time of the occurrences.

Corrective Action

The output module was replaced under Maintenance Work Order 78-1561. CS Pump 1-2 was returned to operability on June 20, 1978.

Failure Data: Containment Spray Pump 1-1 was previously reported to have been inoperable due to defective instrumentation in Licensee Event Report NP-33-77-21.
