e, Inspectors from our Region III (Chicago) office recently completed a

~3 special inspection to review a reportable occurrence involving design

  1. 'i and installation defects associated with the sequencer logic of the Safety Feature Actuation Systas. The findings of this inspection, whfeb included items of noncompliance, were reviewed with the plant Superin-tendent during the inspection and were discussed by Mr. Keppler.

. Ml Our findings indicated that modifications made to the sequencer logic r

portions of the Safety Feature Actuation System were completed on February 12. 1977. and that the startup check-out was completed on -

February 14.1977. This startup check-out did not detect the existing M

wiring armrs. The preoperational test performed on this. system was i

l completed on February 23,1977. and did not indicate any system opera-bility problems. In addition a scheme check was made on the modifica-s tion, which. indicated no wiring errors. However, on June 2. 1978,

during the performance of a surveillance test on a portion of the i

Safety Feature Actuation System, the problem with the sequencer was I

finally identified.

Because of the significance of the defects with the sequencer logic of the Safety Feature Actuation System, the enforcement aspects of this s

i case have been escalated from the Regional Office to Headquarters for

handling.

In addition to the need for corrective actions regarding the specific f

items of noncompliance listed in Appendix A, we are concerned about the r0 9

M effectiveness of your management control over testing activities.

Consequently, in your reply, you should discuss the actions taken or
