The purpose of this' inspection was to obtain an indepth understanding of the
extent and safety significance of' wiring discrepancies identified by an NRC

- Diagnostic Evaluation Team during their evaluation of your facility between
' August 18 and September 15, 1989. Arkansas Power & Light's (AP&L) corrective-

actions for these discrepancies were also examined during this inspection. The

. inspection consisted of selective examination of procedures and representative

records, interviews with personnel, and observations by the NRC inspectors,

The inspection findings are documented in the enclosed inspection report.

' During this inspection, it was found that certain AP&L activities appeared

'to be-in violation of NRC mquirements which resulted in consequences of

potential' safety significance. As discussed in our telephone conversation of

November 6,1989, with your staff, we have' scheduled an enforcement conference,
in your office in Little Rock, Arkansas,' on November 30, 1989. During this

1 conference, you will be given an opportunity to discuss your understanding of

these apparent violations and to explain the circumstences surrounding the

appa mnt failure to maintain the design configuration of safety-related
l
electrical circuits.

We are particularly interested in the electrical contacts found in

- the controls for Service Water Pumps P4A and P40, which appeared to render two
,,

of the three service water pumps inoperable under certain conditions since
l"
iritial startup.
In addition, as discussed with Mr. N. S. Carns of your staff,

during'this enforcement conference you should be prepared to discuss five

Licensee Event Reports (LERs) documented in paragraph 3.d of NRC Inspection

Report 50-313/89-22; 50-368/89-22. These LERs appear to be further examples of

failure to n=irtain plant or design configuration in the past.
