3. Facility Crystal River Nuclear Plant, Unit No. 3
4. Identification of Occurrence Fuel assembly 3A33 fell about five feet during transfer from its shipping container to the inspection station. The area was evacuated and the control center was notified. Preliminary radiation surveys did not show radioactive contamination. Extensive radiation and contamination surveys

, showed that there was no release of radioactive material.

5. Conditions Prior to Occurrence The plant is under construction.
6. Description of Occurrence While transferring the fifty-fifth fuel assembly from its shippinS container b-to the inspection location, it fell approximately five feet to the floor.
7. Designation of Apparent Cause of Occurrence The wire cable for the fuel handling tool pulled out of its swaged fitting.

This resulted in dropping the fuel assembly and fuel handling tool. The cable was certified to support 2400 lbs. and the assembly weighed 1550 lbs.

, The cause of failure is still under study.

8. Analysis of Occurrence l The drop did not cause any radioactive release, exposure to employees, injury to employees or any hazard to the public.

Soms of the fuel pins were bowed, however none of the fuel pins ruptured. Some of the spacer grids were broken. The ?.ower end fitting was bent. Some of welds of the fuel handling tool were cracked.

9. Corrective Action All fuel handling slings will be evaluated and consideration given to elimin-O i

ating the use of swaged connections. Until this evaluation is completed,

'a different type of new fuel handling tool that does not require the use of slings will be employed.
