Dacinnation of Aonaren't Cause of Occurrence: The packing was too loose which allowed leakage out of the packing on DH 2735. The packing had been adjusted
.(loosened) por Maintenance teork order 77-2588 on December 3, 1977, for packing had previously been tightened to a point which caused valve to torque out when it was being operated. The packing gland was loose on LT-RC-14-3 Source Valve which allowed leakage out of the packing. The cause of the loose packing gland is unknown.
