Dear Mr. Keppler:
Toledo Edison acknowledges receipt of your December 3, 1974 letter and enclosure referencing apparent violations of AEC requirements and nonconformances with our quality assurance program.
The items resulted from a site inspection conducted by your office on October 23-25, 1974.

) rollowing a thorough examination of the items of concern, Toledo Edison offers the following information regarding the apparent violations, including corrective actions and steps taken to avoid further repetition.
Item A Instructions or procedures for final inspection of cable trav grounding activities - Criterion V.
Following a thorough review of all design documentation it has been verified that enble tray grounding is not a "Q" item, therefore, the need for approved inspection instructions, procedures or quality control verification is not applicable.
The electrical contractor's Quality Control department terminology utilized not applicable (N/A), in the imprint on the reverse side of the raceway card beside the stamped imprint, " Grounding." The rubber-stamped imprint, " Grounding", on the i
raceway cards resulted when the original issue of the contractor's quality control I
procedure IIP-7749-E14-7a-001, 7-17-73 covering inspection of installation of cable tray hangers and supports, inadvertently included grounding.
Since the procedure was issued and the rubber stamp prepared, the cable tray " Grounding" blank was therefore marked N/A.
The contractor's inspection procedure will be revised during January, 1975 deleting
" Grounding", and the stamped imprint on the back of the raceway cards will also be revised deleting " Grounding."

January 6, 1975 Installation of the cable tray grounding is performed in accordance with approved Bechtel drawings by contractor craftsmen, whose work'is under the surveillance of the craft foreman and Bechtel construction management.
l Item A-2A - Instructions and Procedures for Storage. Shipping and preservation of I
the Emergency Diesel Generator - Criterion XIII Measures have been established for storage and protection of the diesel generators.
These measures are in compliance with approved manufacturer's instructions and procedures:
7749-M-180Q-36-1 (Shipping and Handling Instructions) and 7749-N-180Q-37-1 (Preparation for Storage of a Diesel Generator Unit).

The electrical contractor, Fischbach & Moore /Colgan Electric (F&M/CE), has been directed by construction management memo dated October 29, 1974, to perform generator preservation in accordance with drawing 7749-M-180Q-37-1, including i '
documented monthly gensrator magger testing and generator heater energization verification. The generators were installed in their permanent location upon receipt, and construction management instructions were immediately issued to the electrical contractor to energize the heaters. An informal surveillance was per-i formed on a monthly basis but not documented. The diesel generators are now incorporated in the F&M/CE Monthly Motor Maintenance Log Sheet No. 21 of 31.
Megger readings have been taken for the months of November and December, 1974, and l
are acceptable.

To ensure compliance, Bechtel construction management will inspect and document
!l the condition of the diesel generator equipment, in accordance with the approved

Field Inspection Procedure G-8 and the manufacturer's procedure drawing 7749-M-180Q-37-1, on a weekly basis.
Bechtel Quality Control Engineers will perform 4

and document a monthly review of the electrical contractor's megger testing i-data sheets and maintenance logs.
j It is expected that construction management will achieve full compliance by
.j January 16, 1975 at which time TECo QA and Bechtel QA will perform an audit to assure compliance to the above requirements.

Item A2B - Storage conditions in the Auxiliary Feedwater Pump Area - Criterion XIII

t i Steps have been taken to control the cleanliness of the auxiliary feedwater pump i
room. The Construction Manager has issued a letter to all contractors requiring them to maintain the housekeeping in their various assigned areas.
ITT-Grinnell I
has instructed their superintendents to perform daily cleanup of the auxiliary feedwater pump room on both the day and evening shifts.
To further control discarded electrodes, ITI-Grinnell has obtained 5-quart containers which will be used as weld stub buckets. Written instructions will be issued by ITI-Grinnell to their foremen' relating to the required proper use j
of weld stub buckets.
It is anticipated this action will be completed by January 13, 1975.
I 3-January 6,1975 In addition, both Bechtel and ITT-Grinnell QC will continue to conduct documented surveillance activities of housekeeping practices.
Surveillance of the auxiliary feedwater pump room by Bechtel QC on December 31, 1974, verified yiping spools, valves, and hanger materials located in this room were stored in accordance with Bechtel specifications 7749-M-458 and 7749-M-453.
TECo QA and Bechtel QA will perform an audit during the week of January 13, 1975.
