During an NRC inspection conducted August 16 through September 30, 1989, n i
violation of NRC requirements was identified. The violation involved failure to provide timely corrective action.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C t
(1989) (Enforcement Policy), the violation is listed below:
t-10 CFR 50, Appendix B, Criteria XVI, " Corrective Action," states, in part.
- Measures shall be established to assure conditions adverse to quality such as failures, malfunctions, deficiencies, deviations, defective i
material and equipment, and nonconformances are promptly identified and corrected.
5 Paragraph 16.2.2 of the Arkansas Power & Light Company's Quality Assurance Manual for Operations requires that cognizant supervisors review discrepancies discovered during the course of station operations and take j
appropriate action to resolve the discrepancies and evaluate their safety significance.
Contrary to the above, the inspectors observed two examples of failure to promptly correct previously identified deficiencies. The examples are as follows:
1.
On May 19 1988, the licensee installed a temporary modification in theUnit$letdownsystemwhichreplacedanASMEClass2reliefvalve with an unqualified valve of the s6me type. The temporary

modification was performed to allow repair of the ASME valve. To date, no repairs have been made and the unqualified valve has i
remained in place for over 16 months.
2.
On July 2, 1989, the licensee identified that the automatic reset function of the Unit 2 Channel "Dd pressurizer low pressure trip function was inoperative. This equipment malfunction had been previously identified and a job request written; however, no i
mechanism to insure completed corrective action prior to the next plant shutdown had been established and no ev610ation of safety significance had occurred.
