4 The Cleveland Electric Illuminating License No. NPF-58 Company Docket No. 50-440 Perry Nuclear Power Plant
.As a result of the inspection conducted on August 11 through October 11, 1989, and in accordance with 10 CFR Part 2, Appendix C - General Statement of Policy and Procedure for NRC Enforcement Actions (1989), the following violation was identified:,
i Technical Specification 6.8.1.a requires that written procedures be established, implemented, and maintained for surveillance tests.
Contrary to the above:
L On September 25, 1989, while perfonning Surveillance Instruction SVI-G43-T1306, l:
" Suppression Pool Makeup Timer Channel-A Functional / Calibration for 1G43-K1,"
L plant personnel failed to correctly implement Steps 5.1.9 and 5.1.10. The 1
incorrect installation of test leads into the wrong control room panel resulted 1
in failing a control power transfonner for an Emergency Service W&ter discharge
.l valve and the subsequent need to declare affected Division 1 safety related i
p equipment inoperable.
This is a Severity Level IV violation (Supplement I) (30-440/89023-02).
