personnel to follow safety-related procedures, in accordance with the " General i

Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2.

Technical Specifications (TS) Section 6.8.1 requires, in part, that written procedures shall be established, implemented, and maintained, including the applicable procedures recommended in Appendix A of Regulatory Guide (RG) 1.33 Revision 2 (February 1978). Review," Revision 11, and Plant Procedure OPGP03-ZO-0004, " Plant Conduct of Operations," Revision 10 require strict adherence with approved procedures ahd the use of controlled documents which have been verified as the current revision with the latest changes when perfonning operating i

activities.

l Contrary to the above, on September 13, 1989, three examples of failure to follow approved procedurts were observed:

(1) An operator failed to follow safety-related Procedure OPOP08-FH-0003, " Fuel Transfer System "

Revision 0. Steps 7.1.8 and 7.2.9 during the operation of the Unit 1 fuel handling building fuel transfer control console in that the upender hydraulic pump was not turned on and off during each fuel movement cycle, and (2) an operator failed to have an approved, up-to-date copy of Procedure OPOP08-FH-0009, " Core Refueling " Revision 4, at the fuel handling building control console workstation, and (3) an operator failed to follow the precautions of safety-related Procedure OPOP08-FH-0001,

" Refueling Machine Operating Justification," Revision 0. Step 4.3, during the operation of the Unit I reactor containment building refueling machine in that unsecured articles were left on the refueling machine.

This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, Houston Lighting & Power Company is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statenent or explanation in reply, including for each violation:

(1) the reason for the violation if admitted, the corrective steps which have been taken and the results achieved, the corrective steps which will be taken to avoid further violations, and the date when full compliance will te achieved. Where good cause is shown, consideration will be given to extending the response time.
