e' Appendix A NOTICE OF VIOLATION Commonwealth Edison Company License Nos. DPR-39 DPR-48 Based on the inspection conducted on February 7,1980, through March 28, 1980, it appears that certain of your activities were in noncompliance with NRC requirements as noted below.
This item is an infraction.
1.
Technical Specification 3.9.SD requires that positive reactivity changes shall not be made by boron dilution unless the reactor is suberitical by at least 10% A K/K when containment integrity is not intact.
Contrary to the above, on December 4, 1979, Unit I reactor coolant system (RCS) was diluted to less than 10% shutdown without containment integrity intact.
