O APPENDIX A NOTICE OF VIOLATION Duquesne Light Company Docket No. 50-334 Based on the results of an NRC inspection conducted on December 19, 1979 to January 11, 1980, it appears that certain of your~ activities were not conducted in full compliance with the conditions of your NRC License No. OPR-66 as dis-cussed below..These items are deficiencies.

A.

Technical Specification 6.10.1.d states:

"The following records shall be retained for at least five years:

...d. Records of surveillance activi-ties, inspections and calibrations required by these Technical Specifica-tions." Operating Surveillance Test (OST) No. 1.11.1, Safety Injection Pump Test - (ISI-P1A), Revision 7, implements the surveillance require--

ments of Tec'inical Specification 4.5.2.b.2 and serves as the record of performance of the required surveillance activities.

Contrarv to the above, the records of performance of OST 1.11.1 for September 10, 1979 cannot be located at the station and are thus nut being retained as required.

Technical Specification 6.11 states:

" Procedures for personnel radiation protection shall be prepared consistent with the requirements of 10 CFR Part 20 and shall be approved, maintained, and adhered to for all opera-tions involving personnel radiation exposure." The BVPS Radcon Manual, Appendix 10, Radcon Records and Forms, Part II.B.3, Radcon Instrument

Deficiency Log, Issue 1, provides instructions for recording Radcon instru-ment deficiencies, control of deficient instruments, and administration of Out of Service (0.0.S.) stickers and states, in part, "...B.II.3.c.2)

Contrary to the above, on January 4, 1980, Rtdiological Control Procedures were not adhered to in that 0.0.S. Sticker N';. 3-362 for a Radcon RM-14 Ratemeter (Frisker) was found discarded in the Primary Auxiliary Building, the associated instrument having been returned to service without the required log entries having been completed.
