This is a violation of a limiting condition for oper tion as per Technical Specificaticn 3 5.B.1.

This event is considered to be a reportable occurrence as defined in the Techni-cal Specifications, paragraph 6.9.2.b(4).

Conditions Prior to Occurrence The plant was shutdown for a refueling / maintenance outage.

The reactor was subcritical.

The reactor mode switch was locked in refuel.

The cavity was flooded.

Descriotion of Occurrence On Thursday, April 3,1980, an operator found the piston rod on the closure mechanism for the Reactor Building ventilation automatic isolation valve (V-28-39) broken.

The valve was secured in the closed position as required by the Technical Specification. A Job Crder (5054M) was initiated to repair the valve.

Acoarent Cause of Occurrence Piston rod eye stud was broken. The cause of the failure was undetermined.

Analysis of Occurrence Secondary containment is designed to minimize any ground level release of radio-active materials which might result from an accident.

Reportable Occurrence No. 50-219/80-15/3L Page 2 May 7, 1980 provides secondary containment during reactor operation when the drywell is sealed and in service and provides primary containment when the reactor. is shutdown and the drywell is open.

Secondary containment is required during fuel handling operations and whenever work is being performed on the reactor or its connected systems in the Reactor Building, since their operation could result in an inadvertant release of radioactive material.

V-28-39 is one of a set of two air operated valves in series that will close automatically upon a shutdown or isolation signal.

On Monday, April 1, 1980, the valve was surveilled and no problems were detected.

Two days later the va:ve was found inoperable and removed from service.

The safety significance of this event is considered minimal because the inoperability of V-28-39 only removed the redundancy to isolate the system not preventing the (fstem from per-forming its intended function.

Corrective Action

A new piston rod eye stud was fabricated and installed.

The spare parts list will be reviewed to include the recommended amount of spare parts.

Additionally, the piston rod eye studs will be inspected on all the Reactor Building ventila-tion automatic isolation valves.
