Dear Mr. Hancock:
During the February 26, 1980 Crystal River 3 incident, operation of one pressurizer safety valve with a water-solid system resulted from extended operation of the HPI pumps. This operation represents a " test" of sub-cooled water flow through such a valve at high pressure. The valve was not designed for this service. Documentation of the available data would be of value in future safety reviews.
1 At a March 4,1980 Bethesda meeting, you noted that the opening of the safety valve at about 100 psi below the setpoint was attributed to leak-i age prior to the event. Hence, some valve disc damage could be due to i
leaking steam during prior operations. However, if the valve chattered during operation with subcooled water, additional damage may have occurred.
Therefore, we request the following information regarding the subject safety valve:
1.
Provide all data and other information recorded during the incident which could be used to derive values for the flow rates through the valve and fluid conditions upstream of the valve.
Include, for example, injection flow rates, system pressures and temperatures, including pressurizer temperature and information pertinent to letdown flow.
2.
We were informed during the meetings with you in Bethesda, Maryland, that you intend to inspect the safety valve. You are requested to report the results of the inspection.
