Dear Mr. Seidle:

This letter is intended as a follow-up to my letter of January 14, 1980, informing you of a defect in the actuator shaft of a NAMCO-furnished limit switch installed on the Anchor Darling Main Steam Isolation Valve located at Wolf Creek Site.

The defect involved a broken actuator arm shaft installed on a single limit switch. Follow-up analyses and tests performed by the manufacturer, NAMCO, indicates the failure to be most likely the result of excessive forces applied during shipping and/or handling.

The investigation confirmed the structural adequacy of the existing design and further indicated the as-fabric ited unit to be capable of withstanding leads substantially in excess of nominal design loads.

On the basis of these analyses and tests, no generic deficiency is indicated.

The limit suitch supplier is developing guidance, for users of their switc)es, on how to protect switches from shipping and handling damage.
