Dear Mr. O'Reilly:

On March 15, 1980, a report was made under the provisions of 10 CFR 50.55(e) concerning the improper installation of the inboard and outboard shaft fans for both auxiliary feedwater pump motors at North Anna Unit 2.

In accordance with the reporting requirements of 10CFR21, the following information is submitted:

Siemans Allis 4620 Forest Avenue Norwood, Ohio 45212 D. Description of defect, deficiency or failure to comply:

While disassembling the "B" auxiliary feedwater pump motor because of high bearing temperatures detected during motor tests, it was found that the e:taf t fans had been installed improperly. 'Ihe inboard fan was installed in the outboard fan location and the outboard fan was in the inboard location.

This condition was determined by the fan mold numbers and was common to the "A" pump motor as well.

Similar type motors are installed at North Anna Unit No. I on the auxiliary feed pumps, but it has been determined that the fans are installed correctly.

Both auxiliary feedwater pump motors were removed and transported to an outside electrical equipment company for repair of the shaft fans. One of the motors is expected to be returned to the station on March 19, 1980, with the other motor being returned shortly there-after.
