Notice of Violation Based on the results of the NRC inspection August 2h-26,1977, it appears that certain of your activities were not conducted in full compliance with NBC regulations and the conditions of your Facility License Nos. DPR-38, DPR-h7, DPR-55 as indicated below. Item A is an infraction and item B is a deficiency.
A.
Specification 6.h.1 for this facility requires in part that the station be operated and maintained in accordance with approved procedures.
Contrary to this requirement procedure PT/0/A/120/1 vas not followed in the rotation of'the fuel assembly da= aged on August 23, 1977, in the line sean tester.
B.
Facility Technical Specs.fication 6.1.2.1 requires in part j
that temporary changer to procedures be approved by two i
members of the statian staff at least one of whom holds a senior reactor operator's license on the unit affected, and that such procecure changes be reviewed within 7 days by the station ma tager or his designee.
Contrary to the Technical Specification, changes were made in the conduct of Enclosures 9.h and 9.6 in preparation for the Au' gust,-1977 refueling of Unit 1 to the procedure '
OP/0/A/1503/1 without the required approvals and review.
