g APPENDIX A NOTICE OF VIOLATION Philadelphia Electric Company Docket No. 50-352 This refers to the inspection conducted by a representative of the Region I (Philadelphia) office at the Limerick Generating Station, Unit 1, Limerick, Pennsylvania, of activities authorized by NRC License No. CPPR-106.

During this inspection conducted on November 20-30 and December 3-21, 1979, the following apparent items of noncompliance were identified. These items are infractions.

A.

Appendix B, of 10 CFR 50, Criterion VI, states, in part: "... Measures shall be established to control the issuance cf documents, such as instructions, procedures, and drawings, including changes thereto, which prescribe all activities affecting quality..."

The Limerick PSAR, Appendix D Quality Assurance Program, Paragraph D.4.13 states, in part:

...The vendor shall have written procedures governing the control and use of specifications, drawings and inspection procedures..."

Bechtel's subcontract 8031-A-2A, titled " Masonry for the Limerick Generating) Station" dated June 1977 in Section 2 of Exhibit B (Special Conditio'ns states as follows: " Subcontractor shall maintain at the site of work a copy of the drawings and specifications kept current with all changes and modificatio'ns."

Contrary to the above, as of November 30, 1979, the subcontractor did not maintain at the site of work in the reactor enclosure, elevation j

nor was he required to follow or have an approved document control procedure for the erection of Seismic Class I Masonry Walls.

Drawings i

C-426, C-761, and C-762 were not the current revisions.

B.

Appendix B, of 10 CFR 50, Criterion I, states, in part:

...The authority and duties of persons and organizations performing activities affecting the safety-related functions of structures, systems, and components shall be clearly established and delineated in writing..."

The Limerick PSAR, Appendix D, Quality Assurance Program, Paragraph D.4.1 states, in part that: "...Bechtel site subcontractors shall have a quality control organization shich has the authority and responsibility for seeing that the quality control program is established, planned and implemented.

The quality con'rol organization shall delineate its responsibility and duties in writing..."

Contrary to the above, as of November 30,1979,. authority and responsibility for sseing that the quality control program is established, planned, and implemented for the masonry wall subcontractor was not delineated in writing.

C.

Appendix B, of 10 CFR 50, Criterion V, states in part:

"Acti iti s affecting v

e quality...shall be accomplished in accordance with these instructions, procedures or drawings..."

The Limerick PSAR, Appendix B, Quality Assurance Program, Paragraph D.6.4 states, in part, that: "Bechtel Construction Department...is responsible for contruction of the plant to approved engineering specifications, drawings, and procedures..."

Bechtel Power Corporation Job Rule G-5, Revision 30, states in Section 4.2.1, "All requests for permanent distribution of controlled documents must be in writing on a print room request and are to be reviewed / approved by the Project Field Engineer or his designee before it takes effect.

These are the only personnel / stick locations given working copies of the documents; that.is copies which may be referred to during the performance of work."

Contrary to the above, as of December 21, 1979, a file of hanger drawings which were' uncontrolled was referred to during the performance of work. An uncontrolled drawing which was not the current revision was used for the installation of hanger GBB-122-H10.

Appendix B, of 10 CFR 50, Criterion V, states, in part: " Activities affecting

. quality...shall be accomplished in accordance with these instructions, procedures or drawings..."

The Limerick PSAR, Appendix D, Quality Assurance Program, Paragraph D.6.4 states, in part, that:

"Bechtel Construction Department...is responsible for construction of the plant to approved engineerir.g specifications, drawings, and procedures..."

Bechtel Power Corporation Job Rule M-17, Section 5.5.4 states, that, " Prior to installation of a hanger, the area engineer or his designee be verified that material to be installed-is in accordance with the latest drawing revision. Also, in Section 4.7.b it states that, "The area engineer is responsible for initiating a field change request for hangers which cannot be installed as designed or cannot be installed within specifications."

Contrary to the above, as of December 20, 1979, safety related hangers GBB-112-H20, and HBB-120-Hl7 were installed with pins made of materials not in accordance with the latest drawing revision, and there was no evidence that a field change request had been initiated.
