.'.0 D15CitSq!!!.*1 The design basis fuel handling accident is an event which occurs 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> after shutdown and results in the failure of an entire row of fuel rods (14).
The Control Room is notified primarily by the phone communications, which are required by Technical Specifications.
A backup indication would be the refueling machine area rad monitor.
Operator action then evacuates all personnel from Containment and isolates the area.
3.0 SYMnTOMS OF FUEL HANDLING ACCIDENT 3.1 Ma,ior Symptoms:
3.1.1 Communications from the Containment that a fuel handling accident has occurred.
3.1.2 Area radiation monitor RM-7891, located on the Refuel machjne, alarms.
3.2 Other Symptoms:
3.2.1 Containment airborne radiation monitors alarm and if necessary, close the Containment purge valves.
3.2.2 Other Containment area radiation monitors alarm.
4.0 IMMEDIATE ACTIONS 4.1 Sound the Containment evacuation alarm and announce over the PA System, "A fuel handling accident has occurred; all person-nel evacuate the Containment".
4.2 If purge of the Containment is in progress, close the Contain-ment purge valves.
4.3 Refer to Emergency Procedure 2501, Emergency Plan Procedure.
5.0 SUBSEQUENT ACTIONS 5.1 When conditions allow, re-enter the Containment and recover the fuel bundle.
