Gentlemen:
The subject inspection report idetrified one violation of Criterion V of Appendix B to 10 CFR 50. As applicable, our response to the violation

(1) corrective steps which have been taken and results b
achieved; (2) corrective steps which will be taken to avoid further i
non-compliance; and (3) the date when full compliance will be achieved.
The NRC position on the cited violation and corresponding AP4L response is provided below:
NRC Position Based on the results of the NRC inspection conducted February 28-March'4,1977, it appears that certain of your activities were not conducted in full comp 1' ance with your Reactor License or NRC require-i ments as indicated below:
Criterion V of Appendix B to 10 CFR 50 requires that activities affecting quality shall be prescribed by documented procedures and shall be accomplished in accordance with these procedures.
This requirement is further amplified by Section 5 of the licensee's NRC approved Quality Assurance Program.
Quality Control Procedure 1004.19, " Hold, Caution and QC Tagging Procedure", Revision 2, Temporary Change #2, April 26,1976, Section 4.1.1 requires in part that "an operator will remove the equipment from service, align components as required, and-install the card. The person obtaining the clearance and the operator installing the card should then document the verification of l

Contrary to the above, between January 29, 1977 and Febniary 28, 1977, 31 Hold Cards were i'ot verified as having been installed' and three were not veritied as having been removed in the Hold Card Index..

been verified and documented as such in the Hold Card Index.
Immediate action has been taken to rectify hold card verification discrepancies. Training sessions have been initiated to make plant personnel knowledgeable of QCP 1004.19, " Hold, Caution and QC Tagging Procedure", and the requirements stated therein. Also, adherence to QCP 1004.19 has been stressed'.
