Description of Occurrence:

On October 21, 1976, at 1000 hours0.0116 days <br />0.278 hours <br />0.00165 weeks <br />3.805e-4 months <br /> a preventative maintenance check was made by cycling valve, CV-6202, Reactor Building Chilled Water Inlet Isolation Valve. The valve would shut then reopen after approximately 35 seconds.

t Even though the isolation valve was proven inoperable, the check valve (tested April 30, 1976) would prevent any back flow, hence there was no hazard to the health and safety of the 5

public.
