Gentlemen:
On October 20, 1976 and October 21, 1976, two events occurred involving the failure to perform a surveillance test on the off-site power protective relay interlocks and circuitry, and the failure of the Reactor Building Chilled Water Inlet Isolation Valve (CV-6202) to function properly, respec-i tively. Attached find the subject reports in accordance with Technical Specification 6.12.3.2.
