Based on the results of the NRC inspection conducted August 30 - September 2, 1977, it appears that certain of your activities were not conducted in full compliance with your Reactor License or NRC requirements as indicated below:
1.
Criterion V of Appendix B to 10 CFR 50 requires that activities affecting quality shall be prescribed by documented procedures and shall be accomplished in accordance with these procedures. This requirement is further amplified by Section 5 of the licensee's NRC approved Quality Assurance Program.
Quality Control Procedure #1004.23, " Drawing Control," Rev. 3, dated June 20, 1977, Section 7.3 states in part that, "When work begins on the DCR, the current revision of the drawing is marked referencing the DCR in progress and issued per the MDDL
..." Section 8.2 4
states that, "When revised drawings are distributed per MODL, the responsible shop or individual returns the signed transmittal sheet to ANO drawing control indicating their receipt, their files are updated and old revisions are destroyed."
Contrary to the above, none of the drawings associated with DCR #480 were marked and reissued as required. Moreover, the latest revision::
i of three drawings referenced in the DCR were issued but either the superceded drawings were not destroyed or the latest drawings were

apparently not received.
This is an infraction.

The Technical Specificaticn 6.8.1 states in part, " Written procedures shall be established, implemented, and maintained covering the activities referenced below:

The applicable procedures recomended in Appendix "A" of Regulatory Guide 1.33, November 1972...."
Regulatory Guide 1.33, Section A.10, " Bypass of Safety Functions and Jumper Control," is implemented by plant procedure 1005.04, Rev. 3, which states in part, "A verification of removal of a jumper or bypass or the reinstatement of a lifted lead is to be carried out and documented in a similar manner as delineated in Section 6.1.4 concerning the installation of jumper bypasses and lifted leads."
Section 7.1 further states in part, "... The Shift Supervisor is

s 2-Contrary to the above, entries in the Bypass Jumper Log had not been cleared following completion of the associated activities.
It was not evident that the Shift Supervisor's review had occurred on a monthly basis.
This is an infraction.
