an item not in conformance with statements in the Preliminary Safety Analysis

Report as identified below.
I 1.
The Preliminary Safety Analysis Report, Section 5.5.1.1.c. states, with respect to spot radiographs as an aid to quality control of welder's work on the containment liner, that the criterion for radiographic techniques shall be in accordance with paragraph W-51 of Section VIII of the ASME Code. Contrary to this requirement, our inspectors observed a number of radiographs that did not meet the film density requirements of W-51(a)(3) for the weld (example:
vertical veld between places 25B and 25C) and/or the penetrameter had not been properly shimmed for weld reinforcement to limit the variation e
in density from minus 157. to plus 307, as allowed by paragraph W-51(c)(3) of the code.
Picase provide us, within 30 days with your comments concerning this item and any steps which have been or will be taken to correct it and to minimize recurrence, including any appropriate changes that have been or will be made to your quality assurance program.
