sm l During a review of completed surveillance procedures, it was discovered that the gas-7 89 3 03 l chromotograph had been calibrated using the wrong calibration gases contrary to Tech 7 89 .

  • Ql Spec surveillance requirement 4.6.4.1. Redundancy was maintained as the H9 analyzer was 7 89 05 l available and operable. First time occurrence'. The chromotograph was calibrated using 7 U9 g[ the correct gas mixtures and restored operable.

5. Identification of Occu-ence:

Failure to. calibrate a tm '" f regen gas chromotograph with specified gas mixture as prescribed by Techni . .. ification Surveillance Requirement 4.6.4.1.

6. Condition Prior to Oct Mode 1 power operation.
7. Description of Occurrence: i' During a review of the surveillance procedure for calibration of hydrogen gas ~

analyzer SP-730, it was discovered that the gas chromotograph, channel 112, had been calibrated using the wrong calibration gas mixture. 100% hydrogen had been used instead of mixtures containing one volume percent hydrogen, balance nitrogen.

and four volume percent hydrogen, balance nitrogen as specified by Technical

Specifications. The chromotograph was re-calibrated using the correct gas mixtures j and restored operable.

8. Designation of Apparent Cause:

! The Chem / Rad technician failed to follow the surveillance procedure which prescribed-the correct calibration gas mixtures as prescribed by Technical Specifications.

9. Analysis of Occurrence:

The safety and health of the plant and public were not affected,. as a redunda".t H2 analyzer was available and operable. )

10. Corrective Action. j l

Chem / Rad follow-up report F.U.R.77-175 has been posted and reviewed by Chen/Ra4 1 technicians to insure compliance with the surveillance procedure.
