Identification of Occurrence:
An unmonitored radioactive release to the atmosphere exceeding 100 pei/sec contrary to Environmental Technical Specifications 2.4.2.G & K, 5.6.2.A and Standard Technical Specification 3.3.2.1, Table 3.3-4, 6.
- Conditions Prior to Occurrence:
Mode 1 power operation.

Description of Occurrence:
At 10:45 in the progress of purging the Reactor Building, it was discovered that f.
radiation monitor RM-Al was inoperable. This constituted an unmonitored radioactive release to the atmosphere for a period of 22 minutes, at a release rate of 559.5 microcuries per second. The purge was ceroed immediately upon discovery and radiation monitor RM-Al restored cperable.

Designation of Apparent Cause:
The cause of this event was due to a proc: dural inadaquacy in that containment operating procedure OP-417-did not insure RM-Al running while purging the Reactor Building.

Analysis of Occurrence:
The health and safety of the plant and public were not affected as the 559.5 microcurie per second release rate for 22 minutes represented 0.041% of the maximum allowable instantaneous release rate.

Corrective Action:
The Reactor Building operating procedure OP-417 has been revised to ensure operability of radiation monitor RM-Al before commencias Henctor Building purge.
