from a site inspection conducted by your office on March 12-14, 1975.

Following a thorough examination of the items of concern, Toledo Edison herein offers information regarding these items including corrective actions and the dates for corrective action.
DEVIATIONS Item B.1 Contrary to Section 17.2.11 of the FSAR, the Licensee approved the results of test procedure TP 401.01.1 and Phases I and II of TP 401.05.2 and TP 401.06.2 without the review of (1) docu-mentation of test results and (2) the review of test results by appropriate personnel by the Operations Quality Assurance Engineer.
Response
A revision to FSAR Section 17.2.11 will be submitted by June 9, 1975, to clarify that QA will perform surveillance and audit on a selected bases to assure that a technical review of test results and documentation has been performed by designated and qualified personnel.
Item B.2 Contrary to Section 17.2.11 of the FSAR test procedure TP 401.01.1 was utilized to conduct a preoperational test

Response: The Station will submit test procedure TP 401.01.1 to the Manager of Quality Assurance for review and approval by May 5, 1975.
l Item B.3 Contrary to section 17.2.11 of the FSAR a surveillance test procedure (ST 5002.02) was revised and approved for use with-out the approval of the Manager of Quality Assurance.
Response: The intent of FSAR Section 17.2.11 with the limitations of referenced Section 17.2.2.2.2 has been met with respect to procedure ST 5002.02.
FSAR Section 17.2.11 will be revised by June 9, 1975, to clarify QA responsibilities regarding test procedure review and approval.
Item B.4 contrary to Section 17.2.2.2.1 of the FSAR, administrative procedures covering quality affecting activities (AD 1828.03, AD 1828.04 and AD 1827.07) were approved for use without the review and approval of the Manager of Quality Assurance.
Response
The Station has submitted AD 1828.03, AD 1828.04 and AD 1828.07 to the Manager of Quality Assurance for review and approval.
