!j In your lpiter of September 11, 1970, you expressed sosae concern as to the idegacy
- i cf the Quality Assurance Program at the Ocence Nuclear Station pnrticularly as it i
relates to blockage of certain tendon sheaths and failures of strain gages installed i
in the reactor building concrete.
j With respect to the tendon sheath blockage, the Final Safety Analysis lleport,Section I
S.1. 2.1 does state that the poct-tensioning system shall consist of IG2 dome tendons, 17G vertical tendons, and G32 hoop tendons. It is also a fact that one vertical tenden and two adjacent hoop tendon s' ths contain blockages which prevent the installation

of these tendons. This >ame seciton of the FSAll further states that any three adjacent tendons in any of these groups can be lost without significantly affecting the strength of the structure due to the load redistribution capabilities of the shell structure. We are, nevertheless, aware of the importance of assuring that n11 tendons are placed in the building and have tightened up our inspection of this phase of the work. Lietal plugs j
are pulred through each tendon prior to, during, and after concrete placement to assure that each tendon sheath remains oper.
llegarding the failed strain gages, we would comment as follows --

The gages were' purchased by Specification OS-2G7-A, " Specification for
{
Ilcactor BuiMing Instrumentation", dated Alarch 31, 10G7, under quality assurance procedures. Section 1.4.4 of Specification OS-2G7-A listed j
the schedule requirements and stated that the proof test would be performed January 1,1970. Due to unforeseen circumstances, the proof test has been delayed.
i All gageu that were shop w'.terproofed (Those attached to rebars) wer e tested by the supplier and teot results reviewed by Engite cring. In addition, tha cages attached to rebars were lond testcd by Duhe prior to installatica and found to be satisfactory.
