This event is considered to be a reportable occurrence as defined in the Tech-nical Specifications, paragraph 6.9.2.b.3 Conditions Prior to Occurrence The plant was shutdown for a refueling / maintenance outage.

Moderator Temperature, <212*F Rea ctor Pressure, Atmospheric Description of Occurrence During the Investigation of LER 80-17/1T It was discovered that the surveillance requirements for the "one rod free travel interlock" was not performed on a daily basis as required in Technical Specification 4.9.D.I.

The survelliance described was performed prior to fuel handling and weekly thereafter in accordance with Technical Specification 4.9.A, when fuel was being handled. The interlock was proven operable throughout the entire refueling period by perfornance of the requirement of the latter specification.

Apparent C3use of Occurrence Tn' cause of this occurrence is attributed to personnel error.

Failure to per-e form the surveillance is attributed to the failure to include it on the Master Surveillance Schedule and failure to provide a procedure for its performance.

.Page 2 June 27, 1980 Analysis of Occurrence Even though the surveillance was not performed daily as required, the fulfilled requirements of the Technical Specification 4.9. A surveillance proved the inter-lock to be operable throughout the refueling period. Therefore, multiple rod withdrawal, except as described in LER 80-17, could not have occurred. The safety significance of this event is considered minimal.

It should also be noted that performance of this specific surveillance would not have detected the con ( tions in LER 80-17.

Corrective Action

Since the event was discovered after the completion of refueling activities, no immediate corrective action was required.

Corrective action to prevent a reoccurrence of this type event will consist of (1) Placement of all existing Technical Specification surveillance items on the Master Surveillance Schedule (2) Preparation of procedures for those survell-lances for which procedures do not exist, and (3) Provision for a method of ensuring that new requirements are incorporated on the Master Surveillance Schedule and into procedures.
